What We Do

Navigation

The Well-Being HUB Navigation service offers support to community members along the Eastern Shore and through the Musquodoboit Valley who are 16 years
and over. Our service features two Navigators – Mental Wellness Navigator and Navigator for Seniors.

Our Navigators are community links, helping community members access resources, navigate systems, connect with necessary care and other supports. They also provide information about services, practical supports, and connections to community resources.

If you provide a service, resource, or support related to:

  • aging in place
  • unpaid caregiving
  • supporting mental health & wellness

Programming

We offer workshops and information sessions that aim to educate on topics related to mental health & wellness, aging in place, managing chronic disease, and those that address the social determinants of health. Some of our programming is designed to provide the opportunity to connect socially, bringing community members of all ages together to learn new skills and have fun.

Sessions have included:

  • Advance Care Planning , (ACP) with Nova Scotia Health’s (NSH) INSPIRED COPD
  • Meal planning on a budget with plant-based foods with Chef Lynn Howard
  • Caregiver Stress Management with Caregivers Nova Scotia
  • Conflict management for Care Partners with mediator Delphine du Toit
  • Scams Against Seniors with the Better Business Bureau and Service Nova Scotia Internal Protection Unit
  • Paint Events with Artsy Fartsy
  • Grief support groups
  • among others…

Visiting Buddies

Volunteer Visitor Program

This program takes place along the Eastern Shore and through the Musquodoboit Valley where we match volunteers with an older adult who could use some company. The volunteer visits their “match person” in their home or in the community for one to three hours per week.

The matches last for six months, with the option to extend if resources are available.

Visits are social – and can include:

  • outings
  • doing an activity at the client’s home
  • providing company and help with errands or appointments
  • some navigation to resources, etc.

Pairs decide together on a visit schedule that works for both, so the commitment is flexible. Volunteer participation in the Orientation and regular training opportunities are required.

Past Projects

The Well-Being HUB has led and developed initiatives like:

  • The Well-Being HUB Navigation Service, helping to navigate issues relating to aging in place, public and private care, and unpaid caregiving. Now also offering Mental Health and Wellness Navigation for community members aged 16+. (2021-present)
  • Visiting Buddies, a volunteer visitor program matching isolated seniors with a volunteer visitor. (2022-present)
  • Pathways Beyond Barriers, a project that subsidized transportation and emergency food supports for older adults and unpaid care partners. (2022 to 2023)
  • Bridging the Gaps, an initiative funded by the Mental Health Foundation that provided mental help supports, including private counselling and peer support, for older adults and other community members (2021-2022)
  • Building a transferable model for a rural Adult Day Program (ADP) which could be implemented on the Eastern Shore and/or the Musquodoboit Valley. (2021-2023)
